Hypertensive nephropathy, GFR 59.5, serum creatinine 1.7Patient: Hi, doctor. I was diagnosed with hypertensive nephropathy. And from recent renal scan report, my GFR is 59.5 ml/min/1.73m2, and my serum creatinine level is 1.7 mg/L. What is my condition now? Can it stop kidney progression here with exercises and medications?

Doctor: Hello. I am a nephrologist in kidney disease hospital. I get your question, and I know your condition. You must be very worried about your illness, and I hope through my answer can help you. I know you get hypertensive nephropathy, which is a common kidney disease mainly caused by hypertension.

Your GFR is 59.5 ml/min/1.73m2. GFR is the glomerular filtration rate, and we often divide kidney disease into 5 stages depending on GFR. The GFR in stage 2 is 60-89 ml/min/1.73m2, and in stage 3 GFR is 30-59ml/min/1.73m2. For your condition, your GFR is between stage 2 and stage3, which means your kidney function is moderately reduced. If you don’t control your kidney damage, you will quickly develop into stage 3. On the other hand, if you have effective treatment, your GFR will increase obviously.

Your serum creatinine is 1.7 mg/L. As the normal serum creatinine level range is 0.6-1.2 ml/L for man, your serum level is little higher than the normal level. High creatinine level means your reduced kidney function, you need to control your creatinine level effectively.

I hope from the above answer, you can know your condition well. In addition, only with exercises and medication cannot stop the progression of your kidney damage. You need to have both diet change and medical treatment. You need to change your daily diet.

1. Control your high blood pressure

As you are in hypertensive nephropathy, you need to control your hypertension firmly, which will slow down your kidney damage. You need to keep low salt and sodium diet, and you need also limit your fluid intake, when you may have swelling in your body.

2. Limit your protein intake

You need to take proper protein everyday, in your condition, you can take 0.6 g/ every body weight/ day. Taking proper protein can reduce your kidney burden and slow down progression of your kidney damage. You need to take some high-quality protein foods such as milk, egg whites, meat and fish.

3. Take proper fruits and vegetables

You need to take proper fruits and vegetables in order to supply you enough vitamin C, iron, potassium and other nutrients that you may lack. The amount you need to take is depending on your illness and symptoms. You can send your detailed test report to me, then I can tell you what can eat, and what cannot eat.

Above are some diet suggestions, which will help to prevent the deterioration of your kidney failure. You need also to get rid of smoking and drinking alcohol. You can take proper exercise which will improve your immune system.

On the other hand, you need also to have medical treatment for your hypertensive nephropathy. I recommend you to take Chinese medicine, which will repair your damaged kidney tissues and protect the residual kidney function. Chinese medicine has little side-effects on your illness, and it can control your symptoms fundamentally.
